League Charge Notification

If your club has received a notification of a charge via the leagues online billing system and DO NOT accept the charge given then a Charge Notification Form MUST be completed by the club secretary.

Please note: Payment MUST be made for the charges listed in the notification and you MUST add the appeal fee of £25.00 to the charge(s).

The charge(s) and appeal fee must be paid by bank transfer prior to submitting this form.

The league will review the evidence supplied and respond to the club.
