LEAGUE APPLICATION FORM

In Accordance with Rule 2, We wish to notify the League that it is our intention to continue playing in the league or withdraw from the league or to apply to join the league as a new club.

If your intention is not to rejoin the league then you must notify the league secretary by using this form and selecting withdrawing from league.

This form should be completed in full by your Club Secretary. You will need all club age group information to hand before filling out the form.

PLEASE make sure that your age group team names are the same as your affiliation with Somerset FA. All teams need be affiliated on your club portal.

Please note: Any new club joining the league will be contacted before the AGM in June. All new clubs will need to provide further information.
